Yes patch game, peform your turn, and inform your opponent to do likewise before starting his move.

However, l once had a opponent who was second player in pbem game send me the file, with instructions to patch up !!!.
No problems encountered throught rest of game.

I think it really would depend on what is in the patch as to any effects you might find.

But the overall concept is everyone should patch when a new patch comes out and you shouldn't assume things are back compatible. So everyone should patch at the same time so nothing funny happens in the file.
