You cannot be fired upon, or be doing anything like patrolling or digging in or expend any movement on anything, if you want to recover losses. You can be adjacent to the enemy, as long as he doesnt shoot at you. I dont think you even have to get a result. As well..you have to be in range of your parent HQ, and not be Broken.

As for disruption, just last night, I multi-assaulted a pile of French and reduced them down to 2 Isolated/Low ammo/Disrupted at Max Fatigue. Their HQ was too far away, yet they were in good order on my next turn.
50% chance to get a morale check....1 in 6 chance to pass...

Note that the distance check to the HQ always passes if you are stacked with it.
