Well, it's stuffed again. I've just gone through the three hour process of windows 10 forcing update 1809 on my machine and at the end of that I again have this slow down issue. The same one Lowlanders fix cured.  Only now the offending update doesn't appear in the list I can uninstall.  This is what the list looks like now. Any ideas much appreciated, Lowlander, or anyone else?

OK. following on from the above. In case anyone else gets a reoccurence. This worked for me. Create a shortcut of the exec, then right click the short cut, select properties, select compatibility, then select as below:
